【摘 要】目的：观察探讨双撕囊联合干性前部玻璃体切割治疗先天性白内障的临床疗效，为临床治疗先天性白内障患者提供更多的理论参考。方法：选取来我院就诊并确诊为的先天性白内障患者36例64眼，对其均采用超声乳化白内障吸除、环形撕除晶状体前后囊膜联合干性前部玻璃体切割术。对其中的20例32眼患者一期囊袋中植入折叠式人工晶状体。观察患者术后前房炎性反应、眼角膜及后囊膜透亮程度。结果：手术后，所有患者前房反应轻微且角膜清亮，多数瞳孔圆形且视轴区清亮。其中有6眼后囊膜切开区呈现薄纱洋浑浊，6眼部分虹膜后粘连，8眼出现瞳孔轻度上移现象。术后并发症情况：10眼葡萄膜炎、1眼后发性白内障，给予相应的处理后均全部治愈。没有出现视网膜脱离以及黄斑囊状水肿等严重并发症。结论：双撕囊联合干性前部玻璃体切割治疗先天性白内障具有显著的临床疗效，不仅可以降低儿童后发性白内障的发生率，还可以维持患者视轴清亮，帮助患者恢复视功能，值得临床推广。

【Abstract】Objective:To investigate the clinical effect of double tear capsule combined with dryanterior vitrectomy in the treatment of congenital cataract. Methods: 36 Casesof congenital cataract patients were selected to our hospital and diagnosedwith congenital cataract in 64 eyes, the using ultrasonic phacoemulsificationand curvilinear capsulorhexis anterior and posterior capsule combined with dryanterior vitrectomy. 20 patients with 32 eyes were implanted with a foldingintraocular lens in a bag. The postoperative anterior chamber inflammation,corneal posterior capsule and bright degree. Results: After operation, allpatients were mild and clear corneal anterior chamber reaction, most of thepupils were round and clear visual axis. There are 6 eyes with posteriorcapsular incision area showing gauze ocean turbidity, 6 eyes with irisposterior synechia, 8 eyes appear the pupil light shift. Postoperativecomplications: 10 eyes, 1 eyes of uveitis after cataract, give correspondingtreatment were all cured. No serious complication of retinal detachment andcystoid macular edema. Conclusion: Double tear sac and dry anterior vitreouscutting treatment for congenital cataract has significant clinical efficacy cannot only reduce children after cataract incidence, can also maintain Los clearof patients, help patients recover visual function, is worth the clinicalpromotion.
