探析中西医结合辨治糖尿病周围神经病变的临床疗效

【摘要】目的：探究中西医结合辨治糖尿病性周围神经病变的临床疗效。方法：选取2013年10月—2015年11月我院收治的糖尿病性周围神经病变患者64例作为研究对象，按照随机原则将其分成治疗组与常规组各32例，给予常规组维生素B₁、甲钴胺片治疗，治疗组在常规组的基础上加入方药益气通痹汤治疗，对比两组的临床疗效。结果：治疗组的显效率43.8%（14/32）与总有效率84.4%（27/32）均显著高于常规组的显效率25.0%（8/32）与总有效率53.1%（17/32）；而治疗组的无效率15.6%（5/32）明显低于常规组46.9%（15/32）；经过8周治疗，两组患者神经传导速度（包括SNCV与MNCV）均有所改善，而治疗组患者正中神经、腓肠神经与腓总神经神经传导速度改善情况要优于常规组；在不良反应方面，常规组中出现口干2例，腹胀不适2例，治疗组在常规基础上口服益气通痹汤，其不良反应为口干3例，出现腹胀不适1例，与常规组相比，差异不明显（P＞0.05）。结论：益气通痹汤对治疗糖尿病周围神经病变有着较好的临床疗效，可以明显改善糖尿病性周围神经病变患者的肢体麻木、针刺痛及烧灼感等症状，能有效改善糖尿病周围神经病变患者的运动神经传导速度及感觉神经传导速度，并且不良反应少，具有较高的安全性。

【Abstract】Objective：To investigate the clinical effect of combinationof TCM and western medicine treatment in treating diabetic peripheral neuropathy. Methods: 64 cases of patientswith diabetic peripheral neuropathy, admitted to our hospital from October 2013to November 2015, were selected as research objects and randomly divided intotwo groups, with 32 cases in each group. The control group was given routinetreatment of Vitamin B1 and Mecobalamin Tablets while the reatment group receivedYiqi Tongbi Decoction on the basis of the routine treatment. The clinicalefficacy of the two groups was compared. Results: The explicit efficiency andthe total effective rate of the treatment group was 43.8%(14/32) and 84.4% (27/32) respectively, which was significantly higher than that of the controlgroup, which was 25.0% (8/32) and 53.1% (17/32) respectively; the ineffective rate of thetreatment group was 15.6% (5/32), which was significantly lower than that of thecontrol group, 46.9% (15/32); After 8 weeks treatment, the nerve conduction velocity(including SNCV and MNCV) of both groups improved, the improvement of the nerveconduction velocity of nervi medianus, nervi suralis and nervus peroneuscommunis of the treatment group was significantly better than that of thecontrol group; As for the adverse reaction, there was 2 cases of dry mouth and2 cases abdominal distension discomfort in the control group and there was 3cases of dry mouth and 1 case abdominal distension discomfort in the treatmentgroup, the difference was of no statistical significance (P＞0.05).Conclusion: Yiqi Tongbi Decoction had better clinical effectin treating diabetic peripheral neuropathy; it could relieve the symptoms of mumbnessof the limbs, tingling and burning sensation of patients with diabeticperipheral neuropathy effectively and improve the motor nerve conductionvelocity and sensory nerve conduction velocity of patients with diabeticperipheral neuropathy significantly, besides, it had little adverse reactionand was of high security.
