锁定钢板内固定联合髂骨髓内移植与单纯内固定治疗肱骨近端骨折的疗效对比

【摘  要】目的：探讨和研究锁定钢板内固定联合髂骨髓内移植与单纯内固定治疗肱骨近端骨折的疗效。方法：研究对象选取为2013年5月—2014明年9月我院收治的60例肱骨近端骨折患者，随机分为观察组和对照组各30例，两组患者在一般资料对比上显著无统计学差异，对照组采用单纯内固定治疗，观察组则采用锁定钢板内固定联合髂骨髓内移植治疗，对比两组患者的骨折愈合时间、肩臂手功能障碍评分（DASH）、肱骨头高度丢失、Neer评分及并发症情况。结果：观察组患者的骨折愈合时间、DASH评分及肱骨头高度丢失情况均显著优于对照组，差异具有统计学意义（P＜0.05）；观察组患者的Neer评分优良率为93.3%，显著高于对照组的76.7%，差异具有统计学意义（P＜0.05）；观察组术后并发症发生率为3.3%，显著低于对照组的36.7%，差异具有统计学意义（P＜0.05）。结论：采用锁定钢板内固定联合髂骨髓内移植治疗肱骨近端骨折的疗效明显优于单纯采用锁定钢板内固定治疗，前者能够加速患者的骨折愈合，确保关节功能恢复，减少术后并发症的发生，值得临床推广。

【Abstract】Objective: To study and research the curative effectsand contrasts of joint locking plate internal fixation, with bone marrowtransplant and simple internal fixation in treatment of proximal humeralfractures . Methods: 60 patients were chosen from our hospital between May,2013 to Sept. 2014 with proximal humeral fractures and were divided intoobservation group and control group with 30 cases each, two groups of patientshad no significant differences in general data contrast, control group adoptspure internal fixation treatment, observation group adopts bone marrowtransplantation with locking plate internal fixation treatment. The two groupspatients were after the treatments on the basis of fracture healing time,JianBei hand dysfunction score (DASH), humerus head height loss, Neer scoresand complications. Results: observation group of patients had significantlybetter fracture healing time, DASH score and humerus head height loss situationwere than the control group, the difference was statistically significant (P<0.05); Neer rating of observationgroup patients was 93.3%, and was significantly higher than that of controlgroup 76.7%, the difference was statistically significant (P<0.05); the incidence of postoperative complications in theobservation group was 3.3% which was significantly less than 36.7% of thecontrol group, the difference was statistically significant (P<0.05).Conclusion: using jointlocking plate internal fixation and bone marrow transplantation in treatment ofproximal humeral fractures curative effect is superior to pure with lockingplate internal fixation treatment, the former can accelerate fracture healingin patients with, ensure the joint function recovery, reduce the occurrence ofpostoperative complications, which is worthy of clinical promotion andapplication.
