【摘 要】目的：分析老年髋关节骨折内固定治疗失败后运用人工髋关节置换术治疗的效果。方法：研究我院在2013年5月—2015年5月接诊的40例老年髋关节骨折内固定治疗失败患者，其中股骨转子间骨折18例，股骨颈骨折22例，其中采用全髋关节置换者为24例，双极人工股骨头置换者16例。观察治疗后各效果指标。结果：在治疗半年后，髋关节Harris评估中，优为15例，良为18例，可为5例，差为2例，优良率为82.5%。术前髋关节Harris评分为（42±7）分，术后半年评分为（86±9）分；所有患者中心肌梗死和肺部感染各1例死亡。结论：老年髋关节骨折内固定治疗失败后运用人工髋关节置换术治疗可以有效的保证治疗优良率，加快术后恢复。

【Abstract】Objective: To analyze failure in elderly hip fracturefixation after the treatment usingartificial hip replacement therapy. Methods: The hospital recorded total 40cases of elderly patients with hip fracture fixation failure from May 2013 toMay 2015. Among those 40 cases: 18 cases had intertrochanteric fracture and 22cases had femoral neck fracture where 24 cases were treated with the use of total hip replacement and 16 cases with bipolar hemiarthroplasty.Theperformance metrics were observed after treatment. Results: After treatment forsix months Harris hip evaluation was carried and the results showed that, 15cases were excellent, 18 cases were good and 5 cases were satisfactory and the results were bad in 2 cases, The goodrate was 82.5%.Preoperative Harris hip score was (42 ± 7) points, after six months the score was (86 ± 9) points; amongall patients of myocardial infarction and pulmonary infection 1 patient died.Conclusion: in elderly hip fracture fixation failure using hip arthroplasty fortreatment can effectively guarantee excellent rate, to achieve faster surgeryrecovery effect.
