【摘 要】目的：探讨心理干预联合艾司西酞普兰治疗重度抑郁症的疗效和安全性。方法：将120例患者随机分为对照组和研究组，对照组给予艾司西酞普兰治疗，研究组在艾司西酞普兰治疗的同时进行针对性的心理干预，疗程均为8周，治疗前及治疗后第1、2、4、6、8周末采用汉密尔顿抑郁量表（HAMD）进行疗效评定，用症状量表（TESS）评定用药不良反应。治疗前、后分别对患者行血常规、肝功能、肾功能、心电图检查以评价安全性。结果：研究组治疗后第2周末起HAMD量表评分较治疗前显著下降（P＜0.05，P＜0.01），对照组治疗后第4周末起HAMD量表评分较治疗前显著下降（P＜0.05，P＜0.01）。治疗后第2、4、6周末，两组间比较有显著性差异（P＜0.05），治疗后第8周末，两组间比较无显著性差异（P＞0.05）。两组TESS评分比较差异无统计学意义（P＞0.05）。结论：心理干预联合艾司西酞普兰能显著提高重度抑郁症的临床疗效，安全性高，依从性好。

【Abstract】Objective: To investigate the efficacy andsafety of psychological intervention combined with Escitalopram in thetreatment of severe depression. Methods:120 patients were randomly divided intocontrol group and study group, the control group were given Escitalopramtreatment, at the same time in the study group at Escitalopram treatment wasprovided along with psychological intervention, treatment was carried for 8weeks, Hamilton Depression Scale (HAMD) was used to assess the efficacy beforetreatment and 1, 2, 4, 6, 8 after treatment, symptom checklist (TESS)evaluation of drug adverse reaction was also carried in the similar manner .For safety evaluation blood routine test, liver function, kidney function,electrocardiogram was investigated respectively before and after the treatment.Results; HAMD scale score were significantly decreased (P<0.05, P<0.01) forthe study group after second weeks of treatment and for the control groupsimilar decrease in HAMD Score was seen after fourth weeks of treatment. Therewas a significant difference between the two groups (P<0.05) after the treatment of 2, 4, 6 over the weekend butafter eighth weeks of treatment there is no significant difference between thetwo groups (P>0.05). Nostatistically significant difference in TESS scores were compared in two groups(P>0.05). Conclusion psychologicalintervention combined with Escitalopram can significantly improve the clinicalefficacy of severe depression, high safety, and good compliance.
