超声检查在妊娠晚期产科性出血中的临床应用

【摘  要】目的：研究总结产前超声检测用于诊断妊娠晚期产科性出血病因的临床意义，为临床妇产科确诊妊娠晚期出血提供可参照依据。方法：本文研究采用回顾性分析法进行。选取我院妇产科2010年2月—2015年2月收治妊娠晚期出血患者56例，作为研究对象。征得院方及患者同意。统计产前检查与产后结果符合度、漏诊率、误诊率。结果：轻度胎盘早剥10例（超声检查漏诊1例），中度胎盘早剥10例（超声检查漏诊1例），重度胎盘早剥4例（超声检查误诊1例），边缘性前置胎盘14例（超声检查漏诊1例），部分胎盘前置13例（超声检查漏诊1例），中央性胎盘前置5例（超声检查误诊1例）。超声检查结果与产后确诊结果符合度为89.3%。结论：产前超声检查诊断妊娠晚期产科性出血具有一定的准确性和临床应用价值，但是存在误诊或漏诊情况，需要结合患者实际病情、不断提高超声检查诊断产科性出血病因的临床经验，提高超声检查结果的可靠性和准确性。

【Abstract】Objective: To study and summarize the clinical significanceof ultrasound examination in the diagnosis of obstetric hemorrhage in latepregnancy and to provide solid evidence for gynecology to diagnose hemorrhagein late pregnancy. Methods: This paper applies a retrospective analysis. 56cases of late pregnancy bleeding patients, admitted to our hospital fromFebruary 2010 to February 2015, were chosen as research subjects, both theconsent of the hospital and patient were achieved. Statistical results ofprenatal and postnatal conformity, the rate of misdiagnosis, misdiagnosis ratewere recorded. Result: There were 10 cases of mild placental abruption(ultrasound examination missed 1 case), 10 cases of moderate placentalabruption (ultrasound examination missed 1 case), 4 cases of severe placentalabruption (ultrasound examination misdiagnosed 1 case), 14 cases of the frontedge placenta (ultrasound examination missed 1 case), 13 cases of partialplacenta previa (ultrasound examination missed 1 case), 5 cases of centralplacenta previa (ultrasound examination misdiagnosed 1 case). The conformityrate of ultrasound examination results and post-natal diagnosis results was89.29%. Conclusion: There was certain accuracy and clinical value in applyingprenatal ultrasound examination to diagnose obstetric hemorrhage in latepregnancy, but there were also misdiagnosed or missed diagnosis, therefore,there was a need to enrich the experience of applying ultrasound examination inthe diagnosis of obstetric hemorrhage in late pregnancy on the basis ofpatients actual situation, with a prospect to improve the reliability andaccuracy of ultrasound examination.
